This form can be submitted at any point prior to the trip, whether that’s well in advance or closer to the outing date. Once submitted, you are free to coordinate the details directly with your Participant.

For TAVF Participants

This form is for PHW-COS Participants who are interested in taking part in a Take A Vet Fishing (TAVF) outing. TAVF is a small, mentor-led fishing experience focused on time on the water, connection, and learning at your own pace, no experience or gear required. By completing this form, you’re letting us know you’re interested and sharing basic availability to help with Mentor matching when opportunities open up. This form can be submitted at any time, and while submission does not guarantee a trip, it helps us make thoughtful matches as Mentors become available. Our goal is to make it easy to raise your hand, easy to connect, and meaningful once you’re on the water.
